Criminal Investigation Intake Form

Let’s take care of the important disclaimer stuff! - please read and check off each box to proceed.

We are a volunteer group of lawyers, former police officers, and others. Our goal is to have crimes connected to the Covid-19 pandemic investigated, and where appropriate prosecuted. We are not the police and cannot give confidential informant privilege. Because of constitutional and common law requirements that defendants be given all relevant information when facing charges, it is important for you to understand that information you provide may be given to third parties when appropriate/required.

CISC supports the Canadian Adverse Event Reporting System (“CAERS”) for the collection of adverse reactions connected to Covid-19 vaccines. By checking the box below, you give CISC permission to share the information provided in this section below to CAERS. The information given above will not be shared with CAERS. Unlike the previous checkboxes, this CAERS checkbox is not mandatory. We just think it is a good idea to share adverse reactions with CAERS so that Canadians have a better understanding of vaccine adverse reactions.

Whether or not you consent to us sharing with CAERS, we need you to answer the questions below for our investigation.
